Blinded
Can't see and automatically fails any ability check that requires sight
Attack rolls against the creature have advantage
The creature's attack rolls have disadvantage

Common sources: Blindness/Deafness spell, Color Spray, certain monster abilities. Removal: Lesser Restoration, spell duration ending, specific cure.
Condition
Frightened
Disadvantage on ability checks and attack rolls while the source of fear is within line of sight
Can't willingly move closer to the source of fear

Common sources: Fear spell, Frightful Presence, dragon abilities, certain undead. Removal: Moving out of line of sight, spell duration ending, Calm Emotions.

Paralyzed
Incapacitated (can't take actions or reactions)
Can't move or speak
Automatically fails STR and DEX saving throws
Attack rolls against the creature have advantage
Any attack that hits is a critical hit if the attacker is within 5 feet

Common sources: Hold Person, Hold Monster, Ghoul's Claws, certain poisons. Removal: Spell duration ending, Lesser Restoration, specific cure.
